Club ONE teams will submit the form below to request entry into a particular tournament; they begin filling fast, and Darin will begin entering tournaments for all entries submitted by midnight November 7.  PLEASE plan to have all entry requests submitted by then.  Younger teams may start submitting as early as October 15.

After submitting, it is possible to then use the back button, and the form will retain all of the data, such as your team name and email, and it will not be necessary to enter it over and over.
