After reading the handbook online, please acknowledge by submitting the form below.

I have fully read, understand, and accept the rules, expectations, policies and responsibilities as stated in the handbook.  I understand that my playing time is a coach’s decision and is based on my performance in practice and my attitude toward my teammates and the game of volleyball.

I also understand my child’s image may be posted online within the club website, Twitter, Facebook, or other similar type of media for the purpose of promoting the activities of the club while the said child is actively engaged in any club activities.

By agreeing to become a member of Club ONE, I agree to live within the Club ONE guidelines.  If Club ONE is not in receipt of this electronically acknowledged agreement, the player will not be allowed to practice.
